Support Services

APM Support service programs provide long-term help to the homeless by providing housing assistance and services that provide the tools necessary for successful independent living. Program participants gain confidence and hope through drug and alcohol counseling, life skills training, language skills development and job experience.

APM has a limited number of housing options available for individuals diagnosed with co-occurring disorders, including substance abuse, mental health, and/or HIV/AIDS. The programs provide individuals comprehensive case management services that tailor action plans to individual needs. In addition, our programs promote resident involvement in the community at large through socialization and recreation opportunities.

  • Supportive Services – provides subsidized housing vouchers to homeless individuals who meet the eligibility requirements from HUD and OSH of homelessness, with single families recently released from prison, severe mental illness, and substance abuse diagnosis and/or with chronic medical conditions/physical disabilities. The program names are Hogar, Hogareño and SERA. This service also provides housing counseling, financial management and resource coordination.
  • Proyecto Escalera – a 24-unit, single room facility for homeless individuals who are complying with a recovery program. The program provides housing and case management under 24-hour supervision. Clients receive referrals to health, employment and group management services as needed.
  • Health Promotion – APM’s Health Promotions programs include the La Casa Project, which provides a variety of support services to persons living with HIV/AIDS, including providing escorts to medical appointments, translation services, case management, temporary shelter, housing counseling, and housekeeping support for clients in permanent housing; and helps HIV/AIDS clients access needed health and social resources.
